ABSTRACT

Introduction@#Polycythemia vera (PV) is Philadelphia chromose (Ph)-negative and chronic myeloproliferative disorder (MPN). Moreover, 0.01-2.6 incidences are diagnosed for every 100,000 population. In Polycythemia vera, guanine on 1849th base of 14th ex-one of 9th hromosome is replaced by thymidine and homozygous state is developed. As a result, valine on 617 of JH2 domain in JAK2 is replaced by fenylalanine and activation of tyrosine kinase is increased. Then, signal pathways such as JAK-STAT, PI3K/Akt and ERK1 are independently activated. The JAK2 V617F mutation is identified in approximately 95% of total polycythemia vera diagnosed cases. Furthermore, the average age group of patients with polycythemia vera is 61 and 10% of them are aged under 40 years and male:female ratio is 1:1. </br>In accordance with World Health Organization criteria of 2016: If three major criterias or first two criteria plus one minor criteria are identified, Polycythemia vera is diagnosed. </br> Although life expectancy rate of PV cases in follow-up is high, is lower in comparison with the general same age group of population and essential thrombocythemia (ET). In accordance with an international and large study (n=1545), the average life expectancy rate was 14.1 years among Polycythemia Vera diagnosed patients. More than half of mortality related to the disorder (53%) was caused by unknown etiology. On the other hand, (36\347, 10.3%), (36\347, 10.3%), (32\347, 9.2%) and (13\347, 3.7%) of the remaining mortality cases were caused by acute leukemia, secondary malignancy, thrombotic complications and cardiac failure, respectively.@*Objective@#We aimed to determine JAK2 V617F gene mutation on patients with suspected polycythemia vera and evaluate the CBC/complete blood count/ parameters and clinical signs. @*Material and method@#The cross sectional study was conducted during 8/Mar/2022 ad 20/May/2022 and 13 individuals with suspected polycythemia vera who received outpatient service at hematology department of State First Central hospital (SFCH) and Mongolia-Japan hospital of MNUMS were included. Ethical approval was received by order (№2022/3-02) of 28/Feb/2022 and research ethics review committee. Each and every participant received informed consents and agreed to participate in the study. The current study was conducted with the support of integrated laboratory of Clinical pathology of State First central hospital. @*Result@#Total of 13 participants were included in the study, 38.4% (n=5) and 61.5% (n=8) of them were male and females, respectively. The mean age group was 58.69±7.7 years. The JAK2 V617F mutation was detected in 92.3% (12/13) of patients. The average age group was 57.8 (52-68) for males and 58.6 (43-71) for females. Complete blood count parameters of males with the JAK2 V617F mutation are shown. </br>By questionnaire, some of clinical signs such as fatigue, pruritus after bath, headache, dizziness, bone and muscle pain, hands and feet peripheral cyanosis, numbness and nocturnal sweating were detected on patient with positive mutation.

ABSTRACT

Background@#Acute appendicitis is an inflammatory disease and acommon cause of acute abdominal pain. The application of the sophisticated automated hematology analyzer more precisely define the morphology changes blood cells. According to this diagnose and treatment control issues are solved by easy and not expensive way. Recent studies haveshown that there is a negative correlation between platelet count (PC) and MPV and that theratio of these two values may be more meaningful. The aim of our study was to investigate thediagnostic value of MPV and the MPV/PC ratio in acute appendicitis. @*Methods@#The patients were divided into two groups based on their histopathological findings: the uncomplicated and complicated appendicitis groups. Leukocyte count, PC, MPV and the MPV/PC ratio were compared all groups.@*Results@#Total of 186 patients, including 119 men, were included in the study. The average age of all patients was 10.21±1.22 years. There was statistically significant difference of some parameters in peripheral blood between the uncomplicated and complicated appendicitis groups in terms of all index. (p<0.001). Leukocyte count had a strong discriminatory property based on the area under curve (AUC) 0.714, (p<0.001). MPV, PC and the MPV/PC ratio had weak discriminatory power with AUC values <0.663. Using receiver operating characteristic (ROC) analysis, the sensitivity and specificity of MPV were 81.56and 45.7%, respectively, and 63.78% and 71.06%, respectively, for the MPV/PC ratio.@*Conclusions@#In our study, MPV and the MPV/PC ratio were useful in the diagnosis of acute appendicitis.

ABSTRACT

@#BACKGROUND. Previous retrospective study from our team showed that RDW significantly increased in chronic heart failure (CHF). OBJECTIVE. The aim of the study was to compare the value of RDW-CV with NT-pro BNP and evaluate sensitivity and specificity of RDW for diagnose of heart failure. MATEREALS AND METHOD. Here, we analyzed 20 patients who were suffering from heart failure and were treated in the department of Cardiology, First Clinical Hospital between in June 2017. All patients in the study group were underwent a complete blood count, immunoassay for NT-proBNP. The ejection fraction was analyzed by ECHO cardiograpy. RESULTS. 85.0% of patients were male and remained was female. RBC and RDW was significantly differentially measured in between CHF groups (р= 0.028 p=0.0001). In control groups without heart failure NT-proBNP was estimated up to 100 pg/ml, whereas it was definitelly increased in patient with CHF (р=0.0001). There was strong correlation between RDW and NT-proBNP (r =0.846, p=0.0001). sensitivity and specificity of RDW was 89%, and 74.3 % respectively in CHF. CONCLUSION. According to the our results there was direct correlation between RDW-CV and heart failure (p=0.035). RDW-CV migh be cost effective and routine laboratoty marker to evaulate CHF.

Introduction@#Base excision repair (BER) is mainly responsible for the correction of small base changes of DNA damage. BER pathway involved many enzymes including OGG1 and XRCC1. The defective DNA repair is associated with an increased risk of various cancers including hematologic malignancies-leukemia and myelodysplastic syndrome (MDS). However, it is deniably these polymorphisms alter the susceptibility and clinical outcome of MDS patients.@*The aim@#This study was to evaluate the impact of polymorphisms in gene encoding one protein of BER system: XRCC1 Arg399Gln in MDS and healthy population.@*Methods@#In this study, we recruited 60 health control group [median 47.9 years, 9 MDS subjects [median 56.6 years] were included in this study. Genotyping was carried out by the polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) technique. Allele and genotype frequencies were calculated by direct counting.@*Result@#The frequencies of genotypes of XRCC1 Arg399Gln were as follows: Arg /Arg 1 (11%), Arg/Gln 6 (66%), Gln/Gln 2 (22%) in MDS and Arg /Arg 18.4%, Arg/Gln40%, Gln/Gln41.6% in health control for XRCC1 Arg399Gln. The result revealed that genotypes Arg399Gln increased the risk of MDS@*In conclusion@#this study is the first to analyze XRCC1 SNPs and their associated risk of MDS in Mongolian samples. To fully understand the role of DNA damage and DNA repair in the MDS, prospective studies are needed and other genes (OGG1 Ser326Cys, MUTYH Gln324His, APE Asp148Glu) of base excision repair pathway should be analyzed.

@#BACKGROUND. Regular blood donation can lead to pre-clinical iron deficiency as well as iron deficiency anemia. With Each donation donors lose 220-250 mg of iron. Early detection of iron deficiency is important for the blood donors and even useful for blood and blood product safety and supply. The research work we studied present Ret-HE to be used to detect the occurrence of iron deficiency eritrony level. Purpose: The aim of this study was to determine Ret-He to have sensitivity and specificity for diagnosing iron deficiency than traditional iron measurements. Materials and methods: We performed a cross sectional and case control study of 156 blood donors who served National Center for Transfusion Medicine. Ret-He, hemoglobin, plasma iron and ferritin were measured using XN2000 Sysmex, and CobasE600 Roche. The statistical analysis was done using One way Anova, Rock curve, Kruskal Wallis test. Results: We examined 64(41.02%) male donors, 92(58.9%) female donors by measurements of Ret-He,hemoglobin, serum, iron and ferritin. Survey participants were 8.33%(n=13) with anemia, 91.67% (n=143) without anemia. In donors with anemia the results were: RBC 4.9*106 u/l, HGB 10.8 g/dl (10;11), serum ferritin 5.2 (4.3; 6.3) mmol, serum iron 4.5 (3.7; 5.8) mmol and Ret-He 25.5 (22; 26) pg. Donors were divided into 3 age groups: group I age was up to 25years, group II was between 26-35 years, group III age criteria was above 35. Group I had serum iron 13.5 (10.; 18.), serum ferritin 41.8 (14; 78), Ret-He 32.2 (30; 33.) RBC 5×106 u/l (4.6;5), HGB14.2g/dl (13.3;14. 2). Group II had serum iron 14.6 (11; 19), serum ferritin 54.1 μg/l (29; 138), Ret-He 32.2pg (31; 33), RBC 5.1×106 u/l(4.7;5.1), HGB14.8 g/dl (13.5;14.8),Group III had serum iron 15.1 umol/l (9; 20), serum ferritin 95.7 μg/l (39; 141), Ret-He 32.7pg (31; 34) , RBC4.9×106 u/l(4.6;4.9), HGB 14.5g/dl(13.8;14.5), respectively. According to a curve (Roc) analysis, AUC of serum iron was 0.0963, serum ferritin 0.909, Ret-He 0.975. The mean Ret-He was 32.3pg (31.3;33.4). The optimal cut off value for the Ret-He was 29,25pg by ROC analysis and are presented along with sensitivity 92.3% and specificity 95.1%. Conclusion: 1. Determining the amount of Ret-He has a better sensitivity and specificity for diagnosing iron deficiency compared to traditional iron measurements. 2. Ret-He has diagnostic indicators that are able to detect the depletion of iron reserves, erythron level. And it need to be used in further clinical practices, as well as doctors should be required to use it for diagnosis and treatment.

BackgroundIn Mongolia scientists have been doing research about skin disease and process of aging. Howeverthere has not enough study material about skin aging process which is the key indicator of aging andhow that affect to Mongolian people.GoalMain purpose is to study correlations between main index of skin type of the body, cardiovascularsystem, biological age, epidermal growth factor and blood plasma melatonin, within the people of Uvsprovincial center as they are relatively healthy people.Material and MethodsResearch works were done using cross sectional study model in years 2015-2016, within randomlychosen 400 citizens of Ulaangom city, counted to be relatively healthy people. Research model includes41 questionnaires, examination and blood plasma samples.Basic indexes of the body, cardiovascular system and respiratory functions were chosen by biomarkers,biological age is determined by V.P.Voytenko’s 4th version method and adaptive capacity assessment isdetermined by R.M.Baevskii’s multi-repeating regression method.Main skin type parameters such as porosity, wrinkles, pigmentation, elastics and aging is determined bythe “BOOMTECH skin diagnosis” digital equipment made in the Republic of Korea.The blood plasma MLT and EGF were measured by the method of ELISA analysis ELISA made in thelaboratory of the “AVITSENNA Science and Technology center”.The study material analysis carried out an analysis using logistic regression analysis and Pearson’scorrelation coefficient with SPSS 23.0 software.ResultThere’s an inverse weak correlation between blood plasma MLT and EGF with body wealth characteristicsof Uvs province citizens. It shows that when the blood plasma MLT and EGF decreases the body weight,body height and body weight index increase tendency.In women the decrease of blood plasma MLT and EGF also decreases the body height and body weightindex.Research on correlation between cardiovascular system and blood plasma MLT and EGF shows thatdecrease of blood plasma MLT causes in increased arterial pressures in both sexual groups, decreasedEGF in women causes decreased arterial pressure.Correlation study between skin main characteristics and blood plasma MLT and EGF shows thatdecreased blood plasma EGF in men resulted in decreased skin wrinkles, porosity, elasticity and skin aging index, in women decreased skin pigmentation and elasticity, and increased skin wrinkles, porosity and aging index in women.Research study of correlation between biological age and adaptive capacity assessment with the blood plasma MLT and EGF shows that decreased MLT and EGF in men results in increased biological age characteristics. In other words, the early aging is caused.The logistical regression analyses made using blood plasma melatonin, skin porosity, biological age,body height and sex as the risk parameters of Pearson coefficient and chi-squared parameter. Above model resulted in skin porosity and blood plasma MLT’s are becoming independent variable assessing the skin aging.Conclusions:1. The decreased MLT and EGF’s are resulted in increased body weight, body height and body weight index in men and decreased body height, body weight index in women.2. Decreased blood plasma MLT resulted in increased arterial pressure in both sexual groups and decreased EGF resulted in decreased arterial pressure in women.3. The decreased blood plasma MLT and EGF results in decreased skin wrinkles, porosity, elasticityand skin aging parameter with the increased skin pigmentation in men, as it results in decreasedskin pigmentation, elasticity and skin porosity parameters with the increased skin aging degree in women.4. When epidermal growth factor and blood plasma melatonin is decreased in men it results in increased biological age, but depends directly to the adaptive capacity assessment.5. Skin porosity and blood plasma melatonin were becoming independent variable to assess skin aging.

Backround: Biological age score plays an essential role in the evalution of aging process and health status. The study was focused on clarifying relavance of health status scores of a population of Khangai mountain region with geographical and ecological specifics of the places they live. Goal: To determine the grades of aging (GA) and biological age score (BAS) among population (30-69 years) of Zavkhan aimags.Materials and Methods: We examined 1901 men and women aged 30-69 years old in Zavkhan aimags in 2012 and healthy subjects were assessed with physical, physiological variable. The BAS equation was employed to compute the individual BAScores for all subjects. Quantative data has been processed with the use of statistical data processing software SPSS-19. Results: Health scores of 92.1% of male in the age group of 30-39 years, 68.7% of male in the age group of 40-49 years had health problems (in level IV, V ). In all age groups 70.3% -98.6% had normal adaptation and adaptation functional tension. 54.2- 55.1% of both sex in the ages group 60-69 years had unsatisfactory adaptation.Conclusions: Aging process for women in age groups of 50-59, 60-69 years was significantly delayed or delayed. For men of 30-49 ages aging process was accelerated, for men of 50-69 years it was normal or delayed. Health scores of men in the age group of 30-39, 40-49 years is significantly at a lower level (IV,V) and health scores of women all age groups is significantly at a high level (IIII).

BackroundTerritory of Mongolia is spread over seven natural ecological zones and it has harsh continental climate. Mongolia is one of the most sparsely populated country on the World. The study was done among residents of the Khovd province and its subprovinces as representations of Altai mountain region. The study was focused on clarifying relavance of health status scores of a population with geographical and ecologicalspecifics of the places they live.Goal:To determine BA, grades of aging and health status scores among population (30-69 years) of mountain regions.Materials and Methods:In total 1119 persons were chosen for the research study, which was conducted in 2011. Quantative data of the study has been processed with the use of statistical data processing software SPSS-19.Results14.4% - 94.1% of male in all age groups had health problems. In all age groups 55.9%-85.5% of male and 43.9%-78.4% of female had adaptation functional tension. Conclusions: Aging for women of all age groups was significantly slowed or slowed. Aging for men of 30-49 ages it was accelerated, for men of 50-69 years aging was normal or slightly slowed. The portion of people with adaptation functional tension was greater in all age groups. Most of the women in the age group of 60-69 years had unsatisfactory adaptation. Health status of male is significantly at a lower level compare to female.

In 2006, by the news of National Center of Health Development, morbidity of cardiovascular disease was 501.84 per 10000 people. It’s increased by 2 times than over 10 years.Goal: To study and to determine stereometric research, mathematic modeling and histologicalcharacteristic on cardiomyocyte of arterial hypertension (AH), acute myocardial infarction (AMI) and chronic rheumatoid fever (CRF).Materials and Methods: It was prepared sections for histometric materials from muscular of left ventricle. Linear measurement and number of cardiomyocyte per area of materials were carried out by computermicroscope “Leica” with program Diskus 3.2 version from German. On linear measurement of AH, AMI and CRF were processed by mathematic modeling. On ratio of cytoplasm and nucleus and histological characteristics were analyzed by chi-square testResults: Results of stereometric research on cardiomyocyte On AH volume of cardiomyocyte was 5788.22±40.99mkm3, volume of the nucleus - 806.47±12.86mkm3, ratio cytoplasm and nucleus 1:7 and number of cells per area was 10.On AMI volume of cardiomyocyte was 3240.94±48.96mkm3, volume of the nucleus - 693.85±12.76mkm3, ratio cytoplasm and nucleus 1:5 and number of cells per area was 16. On CRF volume of cardiomyocyte was 5341.06±63.4mkm3, volume of the nucleus - 801.5±15.96mkm3, ratio cytoplasm and nucleus 1:7 and number of cells per area was 11. In comparatively healthy condition at 20 to 30 years old population of Mongolia the volume of cardiomyocyte was determined 1063.17mkm3, volume of nucleus 406.69mkm3, ratio cytoplasm and nucleus 2.6 and number of cells per area was 31. Result of histological research of myocardial muscle. By the research of the difference between histological characteristic was confirmed that all of this characteristics have real main difference. Conclusions: It was confirmed difference with statistical probability by that determined volume of the cardiomyocyte 5788.22±40.99mkm3 in AH, 3240.94±48.96mkm3 in AMI and 5341.06±63.4mkm3 in CRF. Quantity of cells per area was in AH - 10, in AMI – 16 and in CRF 11, but in comparatively healthy condition it was 31.
